Documents for a child in Ontario

This post is to remind/keep track of what we have done for Zoe in terms of all the legal document. I hope this will be helpful for any parents to be.
Please let me know if we have missed anything.

1 - Health Card
We obtained this temp health card from our hospital. The nurse told us that the official one should arrived within the next 2 weeks.

2 - Registering Zoe, getting a birth certificate, and obtaining a SIN (3 STEPS in one).
Before we leave the hospital, we received a package from them, the package contained a form called "Statement of Live Birth". One should fills this form out within the 30 days of any child's birth. However, based on my search from internet, we would registered Zoe online instead of filling this form.
- Registering a child costs $25.00
- Getting a birth certificate costs $25 for a short version and $35 for a long version. Since we are allow to apply for one version ONLY, we opt for the long version(even tho I don't know the diff).
- Getting a SIN is FREE!!!!!!

3. Filling the form for UCCB (not yet done)

4. Immunization Record
We got this from our pediatrician on the second visit.
